Do you take insurance?
A: I do not file insurance; however, you may call your insurance provider to verify whether I am an approved provider. I may be listed as Chad Smith, or Charles M. Smith.
II. All claims are submitted by counselee. I will provide all requested information the insurance company might require.
What is your fee structure?
A: I operate my practice on a sliding scale fee base. Fees are negotiated at time of initial consultation.
Is there a special fee for Court testimony?
A: Yes, my fee for Court testimony is $100.00 per hour from the moment I arrive at the hearing.
Do you facilitate groups?
A: Yes, I facilitate parenting and anger management groups.
What does ISI represent?
A: ISI is a Scriptural reference found in Proverbs 27:17, which reads “As iron sharpens iron, so one man sharpens another.”
